Case Management Care Plan

Case Management Care Plan (CMCP) is evidence-based, integrated clinical care activities that are patient-specific and ensure that every patient has a coordinated plan of care and services.

Case Manager

Case manager (CM) is the health care professional responsible for formulating a coordinated, comprehensive plan for individuals who need long-term health care supervision. The case manager is responsible to work with all health care providers, the patient, the patient's significant others, third party payers, and health care facilities in order to identify and meet the patient's needs.

CBT

CBT (Cognitive behavioral therapy) is a form of treatment that focuses on examining the relationships between thoughts, feelings and behaviors.

CD

CD (Chemical Dependency) is an addiction to a mood- or mind-altering drug, such as alcohol or cocaine
